Extending study permit - confusion on form checklist

Hello, Im a bit confused here in regards to an extra form (common law union form) if it should be filled out, however its not listed in the documents required in the GICKey account after I filled out the questionnaire.

And on immigration website guide 5552 it says an extra common law form must be filled out with application, however that is not showing up on the required list area where we upload documents.
