Expired medical exams

Good morning all,

just got an email from ottawa asking my wife to pass her medical exams and also asking me to pass them again saying that the ones i did before (dec 2011) expired, the problem is they sent a pdf document for my wife to use for the exam but nothing for me and the old one i have is too old (old adress and single status on it).

Any idea on what should i do ?
