Ex husband don't want to sign IMM5604

Please give me advice,,
i received the inquire of IMM5604 from federal today.

how do I fill it up IMM5604?
I can not track my ex-husband for signing for this form and the judge in Morocco will never give me full custody because the father can visite once a week. the father never comes to visit the baby for 3 years. And now he refuses to sign the paper.
Any solution I am blocked I have 30 days to submit the file.
