Hi, i don't have any sort of employment letter or any sponsorship letter for this program. do i still need to fill this out? if yes, then what all sections does needs to be selected by me? Thanks in advance.

In Page 2, Print Name of Principal Applicant, Date and sign down
In Page 3, check the Last box if you don't have Representative (I declare that I have completed my SINP application ................................................... behalf with the SINP) Then Print name of Principal Applicant, Date and Sign. Spouse signature and Date.
Leave 4th Page blank, but write 2-3 times in the page, between the blank lines N.A.

In Page 2, there is no option to print name of principal Application.There exist only 2 options ;one for sign and other for date.What needs to be done then?

i got ADR:
On the Ethical Conduct Disclosure and Declaration Form
record you have declared the use of a paid representative,
but you have failed to fill out Section C in the Ethical Conduct
Disclosure and Declaration Form. When you have a
representative you must also submit the Use of
Representative federal form .

I do not have representative.

I signed the second page and third page, but i did not circle no representation.(b/3)

pls advise am i missing something while filling the form, why are they asking for representative details.

On page 3 , under option 3 , it says, "Please initial" ? what do we have to do here. Do we have to put our signature here ?

if you name is john smith, putting intials means JS. pls verify from other sources too.
