employment reference letter without company's address

I got my letter from one of my previous employers, while the letter contains no address just the manager's phone number and email.

Can I attach my paystub with the company's address and email to make up for it?

Not ideal. Is the reference letter printed on company letter head, or is it a plain document?

Yes, but their letterhead only has a logo, without any contact info
