We forgot to add her parents on the additional family info :-[ , so they requested to fax it to them what do we include is just the file number on top good enough or should we include address and name? thanks in advance!

A good idea would be to fax the form with all the required info, and just in case send it with a cover letter explaining that you are sending extra info that they requested. In the cover letter be sure to include ALL your personal info (name, address, phone, and case number).
