If I need to make an affidavit then what should be it's format & language . And do I need to make an affidavit from the place of my birth or can it be from the place where I am residing now.

Your help is appreciable.

School certificate (usually 10th standard) has DOB mentioned on it and it is widely accepted. You may use your passport or driving licence as proof as well.

Is it sufficient enough or there anything else like notary stamp paper to be made also.

Other than that what all documents should I start collecting for my PR . I am from IT background will I be elligible for applying to PR as I heard that Canadian govt is restricting entry for IT professionals

It should be fine. List is not yet out...wait till May 4th. You can visit cic.gc.ca for documents required.
