Hello, I had a surname change done in 2015 to reconcile my education documents with my national ID documents and passport in Pakistan.

Up till now (in my successful work and study visa applications), I have never disclosed that because I honestly didn't feel it was important/relevant to disclose. My travel on the old passport with the old name (before 2015) was in 2006 so I hadn't disclosed that as it is more than 10 years ago.

Now for eAPR I am thinking of disclosing my name change. Currently, all my required documents (police certificates, education, work, tax, proof of funds, passports, ID, everything has one consistent name so I don't have a name mismatch problem on my documents). I even got a new birth certificate made in 2015 to match the name, so that's not a mismatch either. I am disclosing my previous.

What documents do I need to provide? I can of course do a notarized affidavit saying this is one and the same person, and show scans of my old passport with the former surname. What else do I need to show? And what should I classify this name change as in the application? One option is 'birth', another is 'other'. I am leaning towards classifying as 'other'.

Any help/advice would be appreciated!

If you changed your name on a Govt. ID, you must have gone through the legal procedure. So you can disclose that.
