Documents not in English or french

Hi everyone
For documents not in english or french,, we should:
Attach the original copy
Attach translation
Attach a copy with the date and signature of certified translator

Is that all or should I add something?? Should I certify it through a lawyer?
Also if I prepared these translations more than 1 year ago ( old date) should I update them or it's ok
