As topic. Does every single space that isn't applicable require that I put "N/A" there?

Like:

All other names:
Family name: N/A
Given names: N/A

or is it fine just to put N/A on the "family name" slot or just leave the entire thing blank?
 
I think N/A just in the family slot name is good enough. Thats what I did, and it was just fine.
 
I put "Not Applicable" in every slot because I typed it and it was as simple as Copy and Paste. However if I were writing it, I would just put "Not Applicable" next to the title/question and a line in the boxes which follow. I believe it is a matter of preference.
 
I combined n/a and drawing a line through the boxes in question. The most important thing is to not leave any empty spaces.
 
it's not recommended to leave any blank spaces, so we put n/a in each empty space.
 
We did as didagirl, and it seems to have been fine.
