Good afternoon,
I made my online application for the CAQ two days ago for a PhD program in Montreal, and i want to send the required documents this week, they need the following:
-The original Declaration, commitments and authorizations form, dated and signed.
-An original recent ID photo, passport format (35 mm x 45 mm), with your name on the back and stapled in the box opposite.
-Legible photocopy of pages of your passport bearing personal information and the photo (well defined). Your passport must be valid.
-Letter of admission from the registrar (or Admissions Office) of the educational establishment that you will attend.
I don't understand the second requirement, what do they exactly mean? and what to put exactly in the blank box to the right of these requirements in the checklist? and my ID is in Arabic, is it acceptable? and my final question is; is it okay not to ask about a financial support proof?
