If my wife (and sponsor) is a Canadian Citizen and we have no children (meaning I am the only one who needs to immigrate), do I still need to fill out "Additional Family Information (IMM 5406)"?

Any advice would be greatly appreciated.
 
Yes, you must fill it out. You also need to write your parents and siblings on the additional family information form.

There is a spot on one of the main forms (can't remember the form number, but I think it is one that needs to be validated) where u list your parents and check off if they are deceased. If you have a sibling that has died, list their info of the additional family form, and for location write deceased.

This is a form that is used for all types of immigration, including visitor visas, etc. I don't think they will look into record checks or anything, but the option and info is there for them, just in case.
 
They don't ask for medicals or background checks but they ask for their names in case you wish to sponsor them later.
