Do I need IMM 5490? Please help.

I and my wife have been married for 15 years. We have 2 children.
Do I need to fill out IMM 5490 for my Family class application?
Please help.

For most questions like these we will need to know if your application is 'inland' or 'outland'. For the 5490, "yes" if your application is 'outland', but I don't see it on the doc's list for 'inland' applications. So it seems it will depend on which your application is.
