Do i have to fill supplementary info form or family info form has replaced it? I have heard supp info form is old now family info form has replaced it? its not on the cic website now also.only family info form is there.

You do not require Supplementary information form. I have filled it but it was returned to me and I was told it has been replaced by Family Information Form.

Bro, I haven't got any affidavit. You can have them from any lawyer, they have it ready with them.
