I can't believe I just noticed this now, but I was looking at the photocopy I took of my husband's Medical Report - Section A (IMM 1017). We sent the application off on July 6th and we are applying outland, United States. I guess due to the closure of the Buffalo office, the DMP didn't know what to put for the Visa office. So both fields "Name of the medical office to which IMM 5419 is sent to" and "Visa Office" were left blank. Do you think this will cause problems? I've seen from others that it looks like all the medical reports are sent to Ottawa, and there is a serial number/barcode on it. Am I worrying too much?
 
It's OK to worry! We all are and will continue to until we are done with the process. Anyways, all medicals are sent to Ottawa and requested electronically... your DMP should have given you a carbon copy with a photo attached to it that was to be sent with your application, right next to the photo was a number and a bar code. this form @ my DMP was triplicate 1 copy went with my medical exam one was attached to a big brown envelope for the x-rays and the last copy went with my app. The receptionist told me at the conclusion of my exam that the results would be sent to Ottawa (I had my exam done before the announcement Buffalo was closing, so they must have always went to Ottawa?) and to either make a copy of the form she had given me or write the number off the top so I'm sure it safe to assume that they use the number to track the medical records.

Yes, the DMP did give us a copy of IMM 1017 with a photo and a number with a barcode next to it to include with our application, so they should be able to track it. After reading so many posts on this forum, I'm just worried any tiny thing could cause a major delay!

We did not get anything like that from the DMP. I've been stressing about this whole medical thing since I sent in our application. Now I'm really worried :o

Out of curiosity, what did the DMP give you to send with your application?
