iam a dentist , and i have no such experience , neither do i have any one on this form who is in army , police etc . what should i do ? write N/A in each of the given boxes .

Yes if anything doesn't apply to you, write N/A. Make sure you leave nothing blank on the forms.

I have this question . If i have to right N/A . will i be writing it in all the given boxes on the form or write it in one box and leave the rest of boxes empty ?
