DESPERATE HELP NEEDED FROM SENIORS

Hi all, I am a new member here. I saw here members are very friendly & informative.

Me & my husband are submitting my full documents to London CHC for final review. They have asked me to submit the Immigration Summary Form of which the link is here: http://www.canadainternational.gc.ca/united_kingdom-royaume_uni/visas/forms_skilled-formulaires_qualifies.aspx?lang=eng

I wanted to ask just one question: WHO NEEDS TO FILL THIS FORM?

is it:

1. only principal applicant?

2. both principal applicant & spouse need to fill the same way the have completed the Additional Information Form for CIO, Sydney?
I mean spouse completes the form as principal applicant once & also main applicant completes the form?
