Hi everyone.

I am a mother of 2 Canadian citizens and a spouse of a Canadian citizen with a multiple entry visitor's visa for 5 years.

We didn't apply for the PR right away as we didn't see it a priority, considering my visa and we can extend indefinitely till it expires.

However, the unexpected happened and we just found out that I am 1month pregnant. We live in Quebec and we are looking for options to get lower the cost of my delivery next year. My first 2 deliveries were C. S. We saw how high the cost here and we are looking at Toronto?

Maybe I can ask for some opinions about my situation. Thanks so much.

You can't "extend indefinitely". Maybe a few years. You will eventually be refused for living in Canada as a visitor and told to leave.

Apply for sponsorship.
 
As Canuck implies apply for PR as soon as possible given worst case you could be pregnant, have your visitor extension refused and be asked to leave the country . Even with a multi entry visitor visa never any guarantee to be allowed back in if CBSA suspect you are living in Canada as a visitor which you are.
 
Assume the first two deliveries were outside Canada. Given your previous 2 C-sections you will likely need another one and will have to pay the 20K if you want to deliver in Canada.
