DECISION MADE on March 19, 2013 but passport not yet recieved

My ECAS was changed to Decision made since 19 March 2013. but i have not yet recieved PPT. I did submit medical, RPRF ,pp and everything in July 2012. pls share your views ... what could be the possible reasons behind this delay in Passport delivery...
