my child original d.o.b is november 2015 but due to certain circumstances i registered her name in june 2016..so her official d.o.b is june 2016,,,will this create any issue during medical exam??

That's a clear misrepresentation which you should beware of and can have adverse consequences. Since you know it is not her correct date of birth, you should immediately get it corrected and obtain the right birth certificate. Remember to be honest in all aspects of your application and your dealings.
