could not find form 5563, plz help

Hello friends
I could not download CAIPS request form 5563, the link given at cic or in this site
http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf
does not work, Although I have PDF reader and I even downloaded 5475 the personal consent form, the above link showed the message
as
"If this message eventually replaced by the proper content of the document, your pdf viewer may not be able to display this type of document"
do you know any other link which can open please

Also need the address where the CAIPS request to be sent ??

works for me.. got Adobe Reader 11.0.02

I had the same problem when using Chrome browser, try using Firefox, Safari or IE.
