Consent for access to information for GCMS notes.

I want to order our GCMS notes for my wife. I currently live in Canada and she is in the Philippines. Will she have to mail me a physical copy of the "consent for an access to information and personal information request" (IMM5744E.pdf) or will a scanned copy do? Snail mail is not fun at all so I hope a scanned and printed copy is adequate. Thanks for your time.
