Hi I have a question while I'm filling out the form.

In the Current country of residence section (7) I need to write down country, status and from-to

I studied in Canada since 2004 to now(2016) but I have a 3 year gap (2012-2015) in between because of the military service, and I came back with the new study permit last year.

Should I write Canada - student - from 2004-2016? or only for the period that my new study permit says (2015-2016)?

Were you in a valid study permit from 2004 - 2012?

Then you completed your military service from 2012 - 2015, and now you had another valid study permit, from 2015 - 2016?

If that's the case, then it should be 2 different periods of education, identified in your application, to be consistent with your permits and your background history.
Obviously you'll have to include the military service history in the appropriate sections, so you'd better include all the details, to avoid any problems.
