Hi,

1-The application should be filled in small or capital letter?

2-The apps IMM1344 and IMM0008, have headers with Applicant Name (left) and Date of Birth (right), should I write the name and DOB on each page header?

3- In the app IMM5406, do I have to write the addresses with no abbreviation?

small or capital letters is fine...prefer capital.
