As a sponsor,

Should I enter my personal income from Jan 1 2013 to Dec 21 2013, or the 12 month preceding the date of my application? ex. Jun 2013 to Jun 2014

Should I fill out additional family information? On top of the form, it says that common law partner (if applicable) and all dependent children 18 years of age must complete their own copy of this form.

Should my common law partner (applicant) be the co-signer of the Application to Sponsor form?

What is the fee for sponsoring just my common law partner now?

As an application,

which immigration office requested for processing this application should i enter?

1. I don't recall exactly how the question(s) are worded, so I'm afraid that I can't answer that one for you.

2. The applicant cannot be their own co-signer.

3. The total cost is $1040:
$75- Fee for the sponsor
$475- Fee for the applicant
$490- RPRF (Permanent Residence fee)

4. This depends on whether you are applying with an Inland application (meaning that you must be in Canada) or an Outland application.
Keep in mind that a person can submit an Outland application, even if they are in Canada (or plan to `visit' their sponsor in Canada).

As the guide states, Write your net personal income for the 12 months preceding the date of your application.

No, you don't fill out the Additional Family Information form.
