Hi Everyone,

My wife got request for medical examination. But on the Section A of
the medical request form, High Commisssion has affixed a wrong picture
( which is of some other applicant).
We notified the High Commission at Delhi via Email, Fax and Courier.
It has been 10 days since we brought their attention to the error
commited by them, but havent heard back from the High Commission. DMP
(Doctor) refused to conduct the examination till the application form
is set right.
Are there any particular process that we should follow to notify High
Commission about the error they committed? Could some body advice me
on this?

Have you actually returned the medical request form? If not, that should be your first step. Send it by courier or registered post so you know it was received and ask that they return it to you with the proper photo so that you can proceed with the medical examination as requested.

Thanks for your response. We scanned the Form and sent then with a covering letter.

But you still kept the original form?? You need to return the original medical form to them so that they can detach the other person's photo and attach your wife's photo.
