Clarifications and questions on IMM0008. Please help!

I have few queries on IMM 0008 and any input regarding that is much appreciated.

Question 10 : Current country of residence: From and To dates
I'm currently residing in Canada as "Worker" using my post graduate work permit but came here as a student 5 years back
Should I fill in the "from date" with the date I entered Canada the very first time for my studies and fill "to date" with the expiration date of my Post Graduate Work Permit? (The form doesn't take the today's date in the "to date" field and throws an error stating that "to date" should be greater than today.

Question 4: Immigration office requested for processing this application:
I answered this as "Case Processing Pilot- Ottawa". Is this correct?

Thanks pmpm! I have a question for you. This is regarding the personal history .. Let's say I took a break from school 2010 summer and during the break I was doing a part time job. Does it make sense write it out like this:

From To Activity Status in Country

2010 - 05 2010 - 08 Summer break from school worker(off campus work permit)

I'm confused about the status in country when you are on a break .. Student or worker?

from first entry to canada, to expiry date of most recent work permit or study permit.

on activity you have to put what you did. if work put work and position, and status OCWP holder. if not unemployed. Study permit holder

You said you did part time work, so write what work you did: employment designation.

I was in US working from 2014 to 2017.

Then gap for 2 years.

Again came to US on 8 March 2019.
Went out of US for 1 week vacation in December 2019
In US currently after the vacation.
Visa ends on 4 Nov 2022.

What should be my start and end dates ?
