I am from the United States.

Some or at least one of the forms request my city / town, in the document I am looking at it is specifically requesting my place of birth and then asks for the country.

I can't picture it being useful at all to know some city without the state.

In my case, would I put Lake Charles and then United States of America, or 'Lake Charles, Louisiana' and then 'United States of America'

Silly question but now that we're getting real close to finishing this off I would love if my tiny questions in my head are resolved before it goes out and I start worrying about it.
 
I put my birth city and then 'United States of America' without the state, and it was fine. I don't think it will make much difference either way--if nothing else, the state you were born in also shows up on your birth certificate.
 
It doesn't matter whatsoever. Remember this is a generic application form and most countries in the world don't have states... just city and country. All they really care about is you are were born in America.. even the city i don't think is very important except maybe to compare to other data to make sure it's consistent.
