I have had no changes to my name whatsoever. In this case what do I write in the box for 'Last name' and Given name(s). Can I leave them blank or write "N/A"?
Also for the 4 options given - Truncated name on immigration document, Minor change in spelling, Legal name change, Significant change in spelling - Can I just leave them blank too?
