I am a bit confused as there are different answers in the forum

14 a. Have you held travel documents and/or passports during five (5) year eligibility period?

Do I have to state the Canadian PR card here?

PR card is just like a visa/entry permit, you can't board a flight without a passport. You just have to state your passport details in q14

I was in US as a student and then on work visa, should those be mentioned in Question 14?

I have mentioned in question 13 (Do you currently, or have you ever help immigration or citizenship status in country or territory other than Canada) that I was in US as student and for work with the dates

No.

Question 14 is about travel documents ( like passport ). Visas and work permits are just kind of permission to enter and stay in the country for serving the intended purpose.
