Had difficult time obtaining and downing the subject forms. Tried different web browsers, MAC and PC. Acrobat reader is up to date.
The "wait" message never went away and the form never downloaded. BUT when I downloaded the "wait" PDF message, walla, the form is stored on the computer!! It was magic lol.This should be explained somewhere as it is very confusing.
If I didn't download the "wait" message, I would have never received the form.
