I am working on submitting my eAPR(ITA Dec 22nd).

1. While writing LOEs is there any specific format which needs to be followed?

2. What is CIC address which I should mention on the LOE.

There is no particular format for LoE and you don't need to mention any addresses.

CIC EE doesn't even have a physical address anymore.
