checklist cover page for new delhi CHC application

Hi,
Which checklist to use as the cover letter. the one mentioned by the New Delhi Visa office specific instructions in the Appendix A or the one issued by CIO.

CIO checklist should be the cover page.
However, since u have to submit all the documents once only in CIO, while submitting visa office documents u have to use visa office checklist as the cover page for the additional documents requested by the visa office.
