Changing status from worker to visitor

hi all

my IEC open work permit expires in June 2017 wihc is also when I cna apply for PR via the family class route. I need to swithc to visotr status when my permit expires. Am I correct this is the application to use

Also for question 3 do I select "Initial temporary resident permit or extension of temporary resident permit" or do I choose "extension of my resident status"

Finally I need to indicate how long I intend to visit Canada and what my purpose is. Do I say that I want to live with my girlfriend untill I get PR or do I say I am visiting a friend for a month?
