Sir, my maiden name Firdous which is mentioned on my B.E. degree, Intermediate Certificate and Matric Certificate.
After getting marriage my name change as : Firdous Siddiqui, Actually my husband surname is : Siddiqui. Therefore I added on my CNIC after my madien name.
My husband not uses his surname on National Identity Card, but on my husband CNIC my father in law name is mentioned as Muhammad Sami Siddiqui Which certify my husband surname.Also on my husband passport his name written as (Given Name : Abdul Basit) and Surname (Siddiqui).
Sir i made an Affidavit which is attested with notary public and on stamp paper in which i mentioned my maiden name Firdous and my present name both are same. Should i need to enclosed CNIC copy my self and husband for reference.
Is it enough for proof.
