Certified Translation Document Requirement for PR Application

Hi I want to clarify the requirement for Documents to count as Certified translation
Do I have to submit these 3 documents in total according to canada.ca:
1 - Certified copy in original language
2 - Translation in English and French of orginal copy
3 - affidavit from translator.
Would appreciate if someone can clarify on this.
