In the additional FAMILY INFORMATION form, do i have to fill out the name in ENGLISH and NATIVE Language too ??????

Or can i just fill out the form in ENGLISH ONLY ?????

The form is pretty clear: "Complete ALL names in English and in your native language (for example, Arabic, Cyrillic, Chinese, Chinese commercial/telegraphic code, Korean, or Japanese characters). If additional space is required attach a separate sheet".

Yes you can write by hand...I did that

you could also install the language plug-in...
