Can't find the form to add new-born to file

I had applied in 2011 , simplified process. Now I need to add a new-born to my file (still in process) but can't find the relevant form on the CIC website. Can anybody please post the link here if they know ?

Also the draft (for CAN 150 dollars, I believe) needs to be addressed to the Canadian High Commission (New Delhi in my case) ?

What other documents need to be sent other than the form,draft, copies of passport and the birth certificate ?

Highly appreciate your reply.

The first one is the generic form and the second is the additional family information form, both of which my husband and I had filled when filing the application.

So it seems, there no special form just for addition of a new born ?
