Here's my situation: I'm completing my medical exams this week. This means my process is almost over (Quebec skilled worker). I'm dissatisfied with my representative, and I really don't think they deserve the last (3rd) instalment, which is roughly $1K. I know I have to fill in a form to cancel that, but my question is: would I have any LEGAL issues if I cancelled the appointment of my representative...? May/can they charge me a fine as a result (arguing that I'm 'breaching the contract', for example)?

Besides filling out the form, what else would I have to do?

Whether you have any legal issues depends entirely on the agreement you made with the representative. If you signed a contract with them and you are breaking it, the rep could very well take legal action against you.

For CIC, you just need to send that form and they will remove the rep from your file.
 
If you have not given any future cheques, don't worry......he cannot do anything to you

He cannot influence your file or case also , as he don't have access (once u cancel him).

Just fill the cancellation form and send it by courier to Visa office giving your direct email id, address, contact number
