Landed in Toronto on September 12th, 2018, with my COPR, and I stayed with a friend. Left Canada on September 19th due to an office project in the U.S.
Apparently there was a letter sent by CIC/IRCC asking for two new photos on November 1st (and I came to know about this only last week, because the letter was never received at the address, and I got this information after enquiring in the IRCC Web Forms.
So I have sent the two photos to CIC/IRCC from U.S yesterday.

Now I have lot of questions (if someone can answer me patiently) :-
1. Will IRCC send the PR Card to the same old address in Canada ? (and later I could take a PRTD from U.S, and enter Canada and collect my PR card)
2. Or can I request them to deliver the card to my address in U.S ? (I could inform IRCC/CIC that I had to leave Canada due to unavoidable circumstances).
3. If PR Card cannot be delivered outside of Canada, should I try changing the address within Canada, for the PR card to be delivered ? (since the letter from November 1st seems to be lost, and I thought it would be better to give the address of another friend of mine who lives in Toronto). In this case too, I could later take a PRTD from U.S, and enter Canada and collect my PR card.
4. I tried checking my PR card status - https://services3.cic.gc.ca/ecas/authenticate.do , but it doesn't give any results for UCI, or App.No. or COPR Number. Can someone guide me on this ?
5. If I want to change the delivery address in Canada, again I will face the issue as in Point 4, and my record cannot be found. So how do I change my address, if I want to ?
 
They won't send it outside Canada. Update a proper Canadian address of friend or relative and ask them to mail it to your US address. You can update your Canadian pr card address by calling cic
 
your PR card was most likely cancelled as you exceeded the 30 day time limit to send new photos.

I am just wondering if there is such a case happens, what are the ways the person can reapply for his/her card? Also, cancelled PR card app is nothing to do with his/her PR status, right?

I think you have to mail in a new application with photos and a 50$ fee. Doesn't affect PR Status.

Regarding question 3, many people is not very successful to find their application in that website. Nether did I. But I did link my PR card application in different way. Try this way:
