CAIPS notes ordering question

Hello

I want to order our CAIPS notes... when we first submitted the application we completed an Authority to Release Personal Information form IMM5475 for my dad (who's in Canada as my partner and I are both out of Canada).

Can he now submit a CAIPS notes request on our behalf? I don't want to have to mail a new original signed release form to Canada before we can submit the request... can we just email my dad a copy of the original release to print out and include with the form?

Also, then is the name on the top of the Access to Information an Personal Information Request the name of the applicant and the information at the bottom (request is bring made by) my dad's information?

A signed copy is fine. And yes, the applicant's information at the top and your dad's at the bottom.
