Business card on the emedical

Hi, I noticed that they staple a doctors card to the emedical paper they give you after you are done your medical. Are you suppoused to leave it on when uploading or do they give it for your own record?

You just need to upload emedical.

But the the doctor's card is tapled onto it pretty hard? Should I try to remove it?
