BOWP - new form IMM5710

Hi guys,

finally I am able to apply for a BOWP... the form IMM5710 is new and in question number 3, "I am applying for the following"; it does not have an option for an open work permit... the only choices I have are the following:

1. work permit same employer
2. Restoration of status
3. initial work permit or new employer
4. temporary resident permit

which one will I choose? Initial work permit or new employer?
