I feel like you are worrying too much about something that hasn't even happened yet. First of all, you said your application is still in process and that your wife isn't pregnant YET. The best course of action for you can only be decided depending on WHEN your wife gets pregnant. Now consider these scenarios:
1) if your wife gets pregnant BEFORE medicals, then you will likely opt not to do xray for your wife upon medical request, which will automatically delay your application until the baby is born and is included in the application.
2) if your wife gets pregnant after medicals are done and before visa is issued, then you will need to let the VO know about it. They will decide whether to grant you visa at the time or wait until the baby is born. If you are granted visa at the time, you will HAVE to land BEFORE the baby is born to avoid cancellation of the visa.
3) if your wife gets pregnant after visa is issued but before you land, then also you will need to let the VO know about the pregnancy. But in this scenario, once the VO acknowledges this info, you can land in Canada with the visa but BEFORE the baby is born.
4) BUT, if at any point before getting visa or landing, the baby is born then the baby HAS to be included in the application. This means your visa issuance will be delayed (if prior to visa being issued) or will be cancelled and reissued (if after visa is issued). The time it takes is unpredictable but can take a few months.
Finally, even if the child is born in Canada, the child would have to be added to your application since it's a change in your family composition regardless of whether they need visa or not.
