No, you just go to your bank and request that certificate from them and they will do it for you with in few minutes.

No specific format. As far as I am concerned these are the issues must be in your certificate.

Bank's Paper which has got bank's Logo
Your name and address
A/C no. and Sort Code
Current balance
The date when you opened the account
The date when letter is printed
Bank's stamp (I certify that this is a true copy)
signature of the manager/the employee who is issuing the letter
Detailed branch address if in case CHC would like to make any verification

I did the same as well and my certificate was accepted.
 
Dear mohan joshi. Banks have almost same specific format for such letters for everyone, they dont accept ur request to mention some other informations. I am 100%sure. All these informations are present in that format which u have mentioned.

Yes, the reason I have explained in detailed so that the applicant, who is asking for a format, will get an idea. Secondly, it is very important to mention when the account was opened. It does not automatically appear on the letter. I have got account with Lloydstsb. The manager wrote by pen on the letter that I can confirm that Mr.M Joshi opened his account at LloydsTsb on 21/06/2005. Then she put a stamp I can confirm that it is a true copy. CHC put emphasis on the date when A/C was opened. If someone has opened a bank account two weeks ago for example and deposited money in the account, eventually, the person will be questioned regarding the source of money.
