For Sec:12 in Background Declaration Form(Addresses), i couldnt remember a few addresses that i have lived in the past and the correct duration. In this scenario please guide me on how can i fill out this section. and also how important that the information has to be accurate

This information has to be as accurate as possible. If you can't remember the exact address, provide the VO with as much
information as possible. Maybe ask friends or family for help or call former landlords.
I checked a few addresses on google maps as well.

It's important.

My suggestion would be to at least enter a town (or as much info as you can provide as to where you lived) along with an explanation letter.

The town was all I could remember for two of my former addresses, but with the help of google maps
I was able to make out the exact address.... :)
 
If you can work out the address, it's great. If you can't, then at minimum put down town / state / country and provide an explanation letter. You won't get into trouble for providing too much information, you could get into trouble however for choosing to omit info.

I guess that varies from individual to individual. For you it might've been easy to recall the addresses,
for others it's a nerve wracking process.
