If you are a Canadian Citizen born in Canada or naturalized you can pass on citizenship to your child. The baby is exempt from the medical. Apply for the baby's certificate of Canadian citizenship and at the same time a Canadian passport for eventual travel to Canada. If you are a PR then the baby is added to your spouse's application as a dependent which also requires submission of passport or other acceptable travel document issued by the relevant country. In this case the baby will undergo a medical. Congrats.

As mentioned by Msafiri, if you are PR, or a canadian by descent, do not let your wife land as PR without listing your child to wife's COPR. You must add your baby to the family sponsor application. This would mean a delay to your wife PR application. If your wife lands as PR without listing baby to her COPR, the baby will forever be ineligible to be sponsored under the family sponsorship.
