1. One B4 per family.
2. The COPR No. The 9-digits 'Doc No.' starting with a "T".

Thanks alot Qorax for your usual help, one more clarification;

Do we need to fill B4 for goods accompanaying as well, accompanying things which we will be taking would be like clothings, shoes and general stuff even for these things do we need to fill the form??
 
The declaration form ( served on the airplane before landing ) is enough for usual accompanying personal effects. For Un-accompanied goods, B4 forms are mandatory to get customs duty exemption.

The regulation says, "YES".
