hello
i'm close to submit my application, but I don't know how to attach the extra sheets I used to answer the questions of the form 5490 where ever I needed more space!
in the instruction it says:

"If you need more space for any section, print out an additional page containing the appropriate section, complete it and submit it along with your application."

does it mean that i have to attach the extra sheets together and put them separately in the application? or it's better to attach them to the form 5490 itself based on the page numbers!?

thank you

Put any additional pages directly behind the accompanying form.

Hello there, can we staple these additional pages to the original form? Or everything has to be loose?
Thank you!

don't staple the pages. cic will remove them anyway. use paperclips to hold papers together.

Be sure to label each sheet with your name (at least) and the form number that these extra pages belong to...because if they are removed from the form by CIC, there's a chance that they could get lost. If someone finds them, they can [hopefully] match them with your file.

Papers containing answer should have a header that indicates:
1. Name of applicant (last name at least)
2. Form#
3. Quesiton#

May also add Page x of x in the footer so that they know how many pages there are for that question, if its a multi page answer.

Thanks everyone!
