- IMM5713 form? - i dont have any family representative why they asking me to fill this form
-IMM 5645 form- what is this form i cannot even open it
proof of relation-visiting my friend for her bay shower how to i give proof of relation here.

please help me finish my application

You and your husband will be applying together, you must fill up this form and assign who the family member representative will be.

-IMM 5645 form- what is this form i cannot even open it
